💊 Post 5: Sacubitril/Valsartan (24mg/26mg) — The Cornerstone of Recovery

The Drug: Sacubitril/Valsartan (24mg/26mg twice daily)

This drug is arguably the most important medication I take daily for my Severe LVSD. Often known by its brand name, Entresto, it belongs to a drug class called Angiotensin Receptor-Neprilysin Inhibitors (ARNIs).

I take the 24mg/26mg dose twice daily. The key to this medication is that it is a two-in-one compound, each part serving a critical function in reversing the damage caused by heart failure.

The Heart Failure Pillar: Stopping the Remodel

Heart failure is often characterized by remodeling—the heart muscle slowly becomes stressed, weak, and enlarged. The body releases hormones that attempt to compensate for the weak heart, but these hormones (like Angiotensin II) ultimately cause strain and damage.

Sacubitril/Valsartan works by:

  1. Valsartan (The Blocker): It blocks the harmful effects of the hormones that cause the heart muscle to stiffen and the blood vessels to narrow.

  2. Sacubitril (The Enhancer): It enhances the action of natural peptides in the body that are beneficial—these peptides actually work to lower blood pressure, reduce fluid, and reduce the load on the heart.

The combination is extremely powerful. Its primary goal is to reduce the risk of hospitalization and death and, ideally, to help reverse some of the heart muscle damage (remodeling) that led to my Severe LVSD (EF 20-25%) in the first place.

Why I Take It: Reclaiming Function

Since starting this medication, I've committed to the twice-daily dosing with absolute discipline. This drug is part of the Quad Therapy (the four pillars of LVSD treatment) and represents my best chance at regaining any lost function and securing long-term stability.

It’s about giving my heart a chemical shield that fights the stress hormones and allows it to rest and potentially heal.

Key Things to Monitor

This is a powerful medication, and my doctors carefully monitor:

  • Blood Pressure: As it lowers blood pressure, careful monitoring is essential, particularly when first starting the medication.

  • Potassium Levels: To ensure they remain stable.

  • Kidney Function: Blood tests are necessary to ensure the kidneys are coping with the drug effectively.
